Background. Different forms of otitis media (OM) lead to hearing loss and disability from childhood. Prevention of recurrent and chronic OM is based on system of OM management in small children. Objective of the study. The purpose of this article is to describe our system of OM management in early childhood. Material and Methods. Children at the age 0-7 years with primary OM. Our management included analysis of risk factors and adequate treatment, otoscopic and functional monitoring of middle ear (ME) in OM prone children, diagnostics, and medical treatment of concomitant pathology, surgical treatment of recurrent and persistent OM and postsurgical noninvasive monitoring. Results. The rationality and effectiveness of the primary treatment for OM increase after the analysis of the risk factors to 46% and 58% respectively. Monitoring of UM in children predisposed to OM improves the accuracy of the prognosis of recurrent and chronic OM. The complex treatment of concomitant pathology increases the curative efficacy and helps to identify persistent OM and independent of other disorders. Modified tympanostomy with adenotomy provides the best results in 97% of cases. Conclusion. Management of OM in children is effective and based on teamwork of family doctors, pediatricians, audiologists and ENT doctors. This approach improves the validity and effectiveness of the treatment of children with a high risk of recurrent and chronic OM |
