Introducere: Mucocelul apendicular (MA) este o patologie rară cu potențial malign redus sau sporit. Cea mai severă complicație a MA este pseudomixomul peritoneal (PMP). Autorii descriu cazurile de neoplasme mucinoase ale apendicelui (ANM) și complicațiile acestora tratate în clinică. Material și metode: Serie de 31 pacienți (22F/9B; vărsta medie 54.61 ani) cu MA, AMN și PMP. Pacienții divizați în două grupuri: I (n = 17,54; 84%) - AMN cu potențial malign redus (LGAMN) și II (n = 14,45; 16%) - cu potențial malign sporit (HGAMN). Pacienții din primul lot au fost operați: apendicectomie izolată (n = 13) sau asociată cu alte intervenții (n = 4). Trei pacienți din al doilea lot au avut intervenții primare (apendicectomie, histerectomie+anexectomie bilaterală, anexectomie bilaterală) și operații repetate pentru PMP: intervenţie citoreductivă cu chimioterapie intraperitoneală hipertermică intra- și postoperatorie (HIPEC, EPIC) (n = 9), hemicolectomie dreaptă+HIPEC+EPIC (n = 4), laparocenteză+EPIC (n = 1). HIPEC și EPIC (5POD) a fost efectuată cu 5FU-750 mg/m2. Rezultate: Follow-up: 71.76±12.32 luni pentru lotul LGAMN și 46.43±7.97 luni pentru lotul HGAMN. În grupul LGAMN, s-a dezvoltat PMP într-un caz vs 14 din grupul HGAMN (5.88% vs 100%; p < 0.0001). În grupul LGAMN nu au fost decese vs 5 decese. În grupul HGAMN (zero vs 35.71%; p = 0.0118). Perioada de timp până la diagnosticarea PMP a fost de 1-168 (medie 34) luni. Concluzii: Datele prezentate sugerează că PMP se dezvoltă nu doar din mucocelul malign și din acest motiv este imperioasă monitorizarea tuturor pacienților cu AMN. Există diferență statistic semnificativă dintre durata supraviețuirii și dezvoltarea PMP în funcție de tipul histologic al AMN.

We describe herein the cases of AM and mucinous neoplasms (AMN) and its complications treated in our unit. Materials and methods: Review of 31 patients (22F/9M; mean age 54.61 years) with AM, AMN and PMP. The patients were divided into two groups: I (n = 17,54; 84%) - low-grade appendiceal mucinous neoplasm (LGAMN) and II (n = 14,45; 16%) - high-grade appendiceal mucinous neoplasm (HGAMN). The patients from the first group have undergone interventions: isolated appendectomy (n = 13) or in conjoint with other surgeries (n = 4). Three patients from the second group have had primary interventions (appendicectomy, hysterectomy+bilateral adnexectomy, bilateral adnexectomy) and repeated surgeries for PMP: cytoreductive surgery with intra- and early postoperative hipertermic intraperitoneal chemotherapy (HIPEC, EPIC) (n = 9), right hemicolectomy+HIPEC+EPIC (n = 4), paracentesis+EPIC (n = 1). The HIPEC and EPIC (5POD) was performed with 5FU-750 mg/m2. Results: Follow-up: 71.76±12.32 months for LGAMN group and 46.43±7.97 months for HGAMN group. In LGAMN group, one case of PMP developed vs 14 from HGAMN group (5.88% vs 100%; p < 0.0001). In LGAMN group no deaths were recorded vs 5 deaths in HGAMN group (zero vs 35.71%; p = 0.0118). The time range until PMP was diagnosed represented 1-168 (mean 34) months. Conclusion: Our data confirm that PMP does not develop only from malignant forms of mucocele, and therefore, all the patients with AMN have to be followed-up. There is a statistically significant difference between the survival duration and occurrence of PMP, depending on the histological type of the AMN.
