Rezumat | ||||||
The pathophysiology of sciatica is a complex event with numerous substances and mechanisms acting at various levels. It seems clear that the intervertebral disc per se has certain biologic effects that contribute directly to these pathophysiologic processes. Nucleus pulposus also seems to sensitize the nerve roots to produce pain when exposed to mechanical deformation. The recent success of human trials with anti-TNF agents strongly supports this interpretation of the neural immune studies summarized in this chapter. Although the pathophysiology of sciatica is far more complex than one might first suspect. In this article, the current knowledge about these mechanisms is reviewed and discussed in relation to the clinical features of lumbar disc herniation. |
