Background. Domestic injuries of pregnant women and the development of complications after childbirth, due to diastasis of the pubic symphysis, are considered as the main reasons for providing orthopedic care to women during childbirth and in the early postpartum period. Aim. To conduct a safe and timely diagnosis of the development of traumatic complications in pregnant women and the postpartum period, to conduct activities aimed at improving treatment outcomes. Material and methods. The study included 16 patients with trauma in the 2-3rd trimester of pregnancy and 4 patients who in the last trimester of pregnancy observed pain in the pubic joint area. Clinical examination and USG examined the ligament apparatus, the bony margin of the pubic bones and the indices of the interpubic fibrocartilaginous disc of the pubic joint. Results. Traumatic injuries of 16 locomotor segments were promptly immobilized with plaster, wounds treated, sutured under anesthesia. In the group of patients with changes in the interpubic fibrocartilaginous disc of the pubic symphysis, according to USG, up to 50%, natural birth is possible, without the risk of complete rupture of the pubic symphysis during birth. In the group of patients with structural changes of more than 80% of the surface of the fibrocartilaginous disc of the pubic symphysis, diastase of the symphysis of 2-3 degrees was subsequently observed, which required the stabilization of the pelvis with external device. Conclusion. Ultrasound of injured segments and pubic symphysis in women, before and after birth, has a high potential for objective assessment of changes after a pelvic injury and postpartum diastase between pubic bones and for determining orthopedic treatment. |
