Rezumat

Introduction. In the early period of alcohol dependence remission there may persist various affective disorders associated with hormonal changes. This modifications reduce the adaptive capacity of the organism, leading to early relapses of the disease in approximately 50% of cases. Background. To study the impact of different emotional disturbances (clinical or subclinical) in correlation with the hormonal profile on the stability of alcohol addiction remission. Material and methods. The study included 155 patients in the early stage of disease remission, namely the first 6 months after treatment. All patients were clinically and paraclinically investigated (plasma levels of hormones: ACTH, cortisol, prolactin) along with psychometric assessement (C.D. Spielberger, Wessmann A.E. Rickes D.F. Questionnaires, W. Zung Scale). Results and discussions. The data obtained showed the presence of affective disorders (anxiety, depression, dysphoria) as well as endocrine disturbances, compared to the same parameters in the control group (healthy males of the same age). Thus, in patients with anxiety prevalence there was detected a statistically significant increase of ACTH (p <0.05) and cortisol (p <0.001), while prolactin plasmatic level did not differ from the reference group (p >0.05). Patients with clinical depression had ACTH level decreased (p <0.001), but the level of other hormones remained statistically unchanged (p >0.05). In patients with dysphoric affective disorder there had been detected a marked decrease in plasmatic levels of prolactin (p <0.001) and cortisol (p <0.02), with no statistically significant deviation as compared to the control group (p >0.05). Conclusion. Psychoendocrine changes detected in the early period of remission in alcohol-dependent patients may well facilitate the onset of early disease relapse.
