Introduction: Variceal bleeding carries a high-mortality rate in patients with liver cirrhosis. Coagulation and fibrinolysis are abnormal in liver cirrhosis. The aim of study: to evaluate the level of D-dimer in pts with esophageal variceal bleeding and to establish its relation to mortality. Materials & Methods: During the last two years, 107 pts (M-67, F-40, mean age - 48.9 ± 1.2 years) with esophageal and gastric variceal bleeding (Child-Pugh score - 8.4 ± 0.2) were treated with different endoscopic methods in our department. The 30-day mortality was 18/107 (16.8%). For D-dimer test, only pts with postviral liver cirrhosis were selected. D-dimer levels were measured upon admission. Quantitative D-dimer detection was performed with NycoCard® D-Dimer Single Test (AXIS-SHIELD PoC AS, Norway) set, using NycoCard® READER II. The normal value was < 0.3 mg/L (<300 ng/ml). Results: Mean D-dimer levels showed significant increase in cirrhotic patients according to Child-Pugh class (A vs. B vs. C): 0.4 ± 0.1 mg/mL (95% CI: 0.11 - 0.65) vs. 1.3 ± 0.4 mg/mL (95% CI: 0.44 - 2.21) vs. 1.4 ± 0.5 mg/mL (95% CI: 0.46 – 2.33), “A” vs. “B” – p<0.05, “A” vs. “C” – p<0.01. There was also a statistically significant (p<0.0001) increase of mean D-dimer levels in patients with liver cirrhosis: survivals vs. nonsurvivals - 0.9 ± 0.25 mg/mL (95% CI: 0.43 – 1.44) vs. 1.8 ± 0.13 mg/mL (95% CI: 1.5 – 2.1). Conclusions: The presence of disfunction in patients with liver cirrhosis is associated with increased plasmatic fibrinolytic activity. High-D-dimer is one of the predictors of death in liver cirrhosis and variceal bleeding. |
