Hepatic steatosis, along with alcohol consumption, co-infections, is a known unfavorable factor of chronic hepatitis (CH) C progression. With the progression of liver fibrosis, portal hypertension also develops. Objective: To study arterial hepatic hemodynamics changes in patients with CH C in association with hepatic steatosis. Materials and methods: We investigated 111 patients with CH C, 56 (50.45 %) – with steatosis and 55 (49.55 %) – without steatosis. The control group included 30 healthy persons. Abdominal ultrasound was performed, with duplex Doppler examination of the portal system; with evaluation of diameter, velocities and blood flow volume of the hepatic and splenic arteries, as well as pulsatility and resistance indexes in these arteries. Results: In patients with CH C with steatosis, significant changes were found in arterial blood flow, especially in women, with concomitant steatosis: flow velocity in hepatic (90.27±3.18 cm/s) and splenic arteries (93.18±8.7 cm/s) was higher than in the control group (77.58±4.70 cm/s, р<0.05 and 69.89±2.73 cm/s; р<0.05 respectively). Pulsatility index in hepatic (1.58±0.09) and splenic arteries (1.19±0.10) was also higher than in the group without steatosis (1.18±0.1; р<0.01 and 0.85±0.04; р<0.01, respectively). In patients with CH C and steatosis a larger size of the liver caudate lobe was found (37.18±1.63 mm) in comparison to patients without fatty liver (31.44±1.78 mm, p<0.05). Conclusions: The presence of hepatic steatosis in patients with chronic hepatitis C has a negative impact on arterial hepatic hemodynamics. |

Odată cu progresarea fibrozei hepatice, se dezvoltă şi hipertensiunea portală. Scopul: studierea modificărilor hemodinamicii arteriale hepatice la bolnavii cu HC C asociată cu steatoză hepatică. Material, metode: au fost examinaţi 111 bolnavi cu HC C, din ei 56 (50,45%) cu steatoză hepatică şi 55 (49,55%) fără steatoză. Lotul martor au constituit 30 persoane practic sănătoase. S-a efectuat examen ultrasonor abdominal, examinare Doppler a sistemului portal cu determinarea diametrului, vitezelor şi volumului fluxului în arterele hepatică şi lienală, indicii de pulsatilitate şi de rezistenţă în arterele respective. Rezultate: La bolnavii cu HC C cu steatoză s-au depistat modificări semnificative în fluxul arterial, în special în lotul de femei cu steatoză asociată: viteza medie în artera hepatică (90,27±3,18 cm/s) şi lienală (93,18±8,7 cm/s) au fost semnificativ mai mari ca în lotul martor (77,58±4,70 cm/s, р<0,05 şi 69,89±2,73 cm/s; р<0,05 respectiv). Indicii de pulsatilitate în artera hepatică (1,58±0,09) şi lienală (1,19±0,10) la fel au fost mai mari comparativ cu lotul fără steatoză (1,18±0,1; р<0,01 şi 0,85±0,04; р<0,01, respectiv). La bolnavii cu HC C şi steatoză hepatică s-au depistat şi dimensiuni mai mari ale lobului caudat hepatic (37,18±1,63 mm) comparativ cu bolnavii fără steatoză (31,44±1,78 mm, p<0,05). Concluzii: Prezenţa steatozei hepatice la bolnavii cu hepatită cronică virală C influenţează negativ hemodinamica arterială hepatică.</p></description> </descriptions> <formats> <format>application/pdf</format> </formats> </resource>
