Background. Congenital supravalvular aortic stenosis (CSAS) occurs in 5-8% of patients with a fixed form of obstruction of the left ventricular ejection tract with a share of 0.05% of total congenital heart malformations Objective of the study. The evaluation of the immediate and long-term results of the various surgical correction tactics of CSAS and the elaboration of the optimal treatment algorithm. Material and Methods. In the years 1992-2020, 20 patients were operated, aged 1-19 years, with SASC, confirmed by ultrasound, aortography or AngioCT. Williams-Beuren syndrome diagnosed in 12 patients, sporadic form - in 8. Results. 25 operations were performed: 11 - Valsalva sinus enlargement plasty, 8 - Doty operation, 1 - Brom plastiy. In 3 patients - and aortic coarctation plasty, in 1 - repeated ascending Ao plasty for restenosis, in 1 patient - mitral replacement. The systolic pressure gradient (SPG) at the stenosis level was determined by the pre-, postoperative transthoracic EcoCG. Postoperative mortality 0. Preoperative SPG was 50 - 145 mm Hg (mean 82.6 + 17mmHg), while in the postoperative period 8-30mmHg (mean 14.6 + 7 mm Hg), without significant changes in long-term period. Conclusion. Plastic surgery in CSAS is effective, durable over time, with complete elimination of the obstruction regardless of the method used. The preoperative investigation protocol should include aortography or angioCT. |
