Introducere: Ligaturarea şi scleroterapia endoscopică (LE); (SE) sunt metode de primă intenţie în hemostaza varicelor esofagiene (VE) hemoragice. LE şi SE sunt dificile în caz de hemoragie variceală activă cu câmp de vizualizare endoscopic limitat. Material şi metodă: Au fost analizate rezultatele utilizării SMAE la 17 pacienţi (b-10, f-7); vârsta medie–52.2±2.1 ani cu ciroză hepatică (HBV, HCV); hemoragie masivă din VE (n=12) sau ulcere esofagiene post LE (n=5). Funcţia hepatică medie: scorul Child-Pugh – 9.5±0.4; scorul MELD – 17.7±1.7. Hemostaza a fost obţinută prin utilizarea SMAE acoperite (SX-ELLA stent Danis, 135 mm×25 mm, ELLA-CS, Hradec-Kralove, Czech Republic). Poziţionarea SMAE a fost verificată prin metode endoscopice şi imagistice. Rezultate: Hemostaza – (17/17, 100%). Media SMAE in situ – 94.3±14.1 ore. Migrarea parţială a stentului – 5/17 (29.4%) – repoziţionare endoscopică. La înlăturarea SMAE au fost efectuate: LE (inele ori mini-loop) (n=7) şi (n=2) respectiv şi SE cu n-butyl-cyanoacrylate (n=1). Mortalitatea la 30 zile – (n=7, 41.1%): insuficienţa hepatică(n=5), VE şi G Sarin I hemoragice (n=1) şi AVC (n=1). Concluzii: SMAE pentru VE hemoragice sunt efective în: (1) hemostaza pentru VE hemoragice; (2) ulcere esofagiene hemoragice post LE; (3) migrarea SMAE este un dezavantaj ce necesită metode de profilaxie; (4) SMAE in situ induce escare a mucoasei esofagiene.

EL and ES present formidable challenge for active EV hemorrhage, especially with limited endoscopic view. Material and methods: The results using SEMS in 17 patients were analyzed (m-10, f-7); mean age– 52.2±2.1 with liver cirrhosis (HBV, HCV); massive EV bleeding (n=12) or post EL esophageal ulcers (n=5). Mean liver function tests: Child-Pugh score – 9.5±0.4; MELD score – 17.7±1.7. Hemostasis was obtained using covered SEMS (SX-ELLA stent Danis, 135 mm×25 mm, ELLA-CS, Hradec-Kralove, Czech Republic). SEMS position was checked by endoscopy and imagistic methods. Results: Hemostasis – (17/17, 100%). Mean time of SEMS in situ was 94.3±14.1 hours. Partial stent migration – 5/17 (29.4%) requiring reposition using endoscopy. Upon stent removal following procedures were performed: EL (rubber bands or mini-loops) (n=7) and (n=2) respectively and ES using n-butyl-cyanoacrylate(n=1). Thirty days lethality – (n=7, 41.1%): liver failure(n=5), EV+gastric Sarin I bleeding(n=1) and stroke(n=1). Conclusions: SEMS in EV bleeding proved efficacy in: (1) hemostasis for active EV bleeding; (2) in case of bleeding esophageal ulcers; (3) disadvantage is SEMS migration, that should dictate prophylaxis methods; (4) SEMS in situ induces pressure necrosis on esophageal mucosa.
