Introducere: Joncțiunea gastro-esofagiană este o zonă anatomică complexă cu funcție inerentă de barieră antireflux. Hernia hiatală (HH) în funcţie de tip şi dimensiuni dereglează această barieră. Scopul studiului: Aprecierea posibilităţilor endoscopiei digestive superioare (EDS) în evaluarea HH. Material şi metodă: Au fost examinaţi 209 pacienţi cu funcţionalitate deficitară a JEG, în proendoscopie şi retroflexie. Endoscop GIF HQ190 (Olympus). Caracterizarea detaliilor mucoasei- regim HD, WLE, Near Focus, NBI+. Distribuţia pacienţilor conform clasificării Hill: grad II (n = 82), grad III (n = 73), grad IV (n = 54). HH a fost definită ca migrarea suprahiatală ireversibilă a JEG până ≤ 20 mm sau migrarea reversibilă a JEG > 20 mm suprahiatal. Toţi pacienţii cu HH au fost examinaţi şi prin radioscopie baritată cu probe de provocare. Rezultate: S-au determinat HH tip I (n=50), tip II (n=7), tip III (n=8). Nu s-au evidenţiat stigmate endoscopice clare pentru HH tip IV (n=4). Corelaţia cu esofagita de reflux: HH tip I în 38(76%), HH tip II - absentă, HH tip III în 2(4%) cazuri. Toate cazurile Hill IV au reprezentat HH, gradul Hill III 15(20,5%) cayuri. Nici un caz Hill II nu a fost HH. Concluzii: EDS diferenţiază cu precizie înaltă funcţionalitatea deficitară a complexului antireflux gastro-esofagian de HH. Aportul EDS în diagnosticarea şi gradaţia esofagitei de reflux, a esofagului Barrett şi AC precoce este superioară radiologiei. Determinarea dimensiunilor HH endoscopic este posibilă şi precisă. EDS nu este informativă în HH tip IV.
Introduction: The gastro-esophageal junction is an anatomically complex area with an inherent antireflux barrier function. Hiatal hernia (HH) disrupts this barrier based on its type and grading. The aim of this study is to recognize the worth of upper gastrointestinal endoscopy (UGE) in determining HH. Materials and methods: A total of 209 patients with problematic functioning of GEJ were examined through proendoscopy and retroflexion - Endoscope GIF HQ190 (Olympus). The detailed characteristics of the mucosa – regime HD, WLE, Near Focus, NBI+. According to the Hill classification: grade II (n = 82), grade III (n = 73), grade IV (n = 54). HH is defined as irreversible superhiatal migration of the GEJ < 20 mm or reversible migration of the GEJ > 20 mm above the diaphragm. All patients with HH were also examined under barium radioscopy with the use of provocation probes. Results: HH type I (n=50), type II (n=7), type III (n=8) were determined. No clear endoscopic stigmata for HH type IV (n=4) were found. Correlation with reflux esophagitis: HH type I in 38(76%), HH type II - absent, HH type III in 2(4%) cases. All Hill IV cases represented HH, Hill III grade - 15(20,5%) cases. In none of the Hill II cases HH. Conclusions: UGE differentiates with great precision the deficient functionality of the HH gastro- esophageal antireflux complex. The contribution of UGE in the diagnosis and gradation of the reflux esophagitis, of Barrett's esophagus, and early AC is superior to radiology techniques. The endoscopic measurement of HH dimensions is possible and precise. UGE is not informative for HH type IV.
