The prostetic valve endocarditis
Infective endocarditis (IE) is a severe, microbial infection of endocardial surfаce, occuring
on intact native heart valves, parts of a valve prothesis or on reconstructed native heart valves; establishes structural failure and systemic embolism. Prosthetic valve endocarditis (PVE) is due to the infection with microorganisms, especially bacteria and fungi; it is observed within the first year of operation, perioperatively and thus nosocomial (early PVE) or as community acquired (late PVE), after one year of operation. The incidence of PVE is 9-20%; in early PVE is accounting 1,4-3,1% and in late PVE 3,2-5,7%. Mortality rate for PVE is maintaining on a top level 23-60%. Thromboembolic events are the most frequent cause of mortality in PVE. Last decade the number of IE in the elder population has been increased considerably, because of predisposing factors: ischemical cardiopathy, diabetes mellitus, infective trophycal ulcers, therapeutic and diagnostic procedures, prolonged venous cathetherism and the presence of prosthethic heart valves. The authors present a clinical case of an elder patient, with early PVE and comorbidities (diabetes mellitus). |
