Diabetic cardiomyopathy (DCMP) has been defined as a distinct clinical entity characterized by the presence of abnormal myocardial performance or structure in the absence of coronary artery disease, hypertension, and significant valvular disease. Metabolic abnormalities (that is hyperglycemia, hyperinsulinemia, and hyperlipemia) can lead directly or indirectly (that is by means of renin-angiotensin system activation, cardiac autonomic neuropathy, alterations in calcium homeostasis) to the morphological myocardial alterations (that is myocardial fibrosis and myocardial hypertrophy). Since DCMP is highly prevalent in the asymptomatic diabetic patients, screening for its presence (evaluation of microalbuminuria, BNP) and diagnosis of the functional myocardial abnormalities (echocardiography and other cardiac imagistic techniques) in diabetic population can lead to prevent the progression to chronic heart failure. Correction of the metabolic abnormalities represents the main therapeutic target and future novel treatment strategies based on the recent
experimental data may lead to improved outcome in patients with DCMP. |
