Rezumat

Introduction. Worldwide, there is a growing body of data that highlights the evolving neurological manifestations in children with COVID-19 infection. The prevalence reported in some studies ranges from 3.8% to 44%. Purpose: to study the type of neurological manifestations in children with COVID-19 infection, to evaluate the diagnosis and the correct management of the disease case in the acute and post-COVID-19 period. Materials and methods. In the framework of an ongoing prospective study, we analyzed patients who presented neurological manifestations associated with the COVID-19 infection, aged from 1 month to 5 years, who were for treatment in the covid sections of the IMSP Mother and Child Institute during the period 01.12.2022-01.02.2023. We divided these manifestations according to their type: specific and non-specific. For the study we used neurological testing, laboratory tests and electrophysiological investigations (electroencephalogram – EEG). Statistical methods used: t-student test and 95CI confidence coefficient. Results. Of the 138 hospitalized children with confirmed COVID-19 infection, only 46 cases (33.3%; 95CI 37.31-29.29) were reported with neurological manifestations. Of these, 26 children (56.5%; 95CI 63.81-49.19) presented specific neurological manifestations, including: toxic-infectious encephalopathy – 18 (39.1%; 95CI 31.9-46.3) cases, seizures – 3 (6.5%; 95CI 2.86-10.14); venous sinus thrombosis – 1 (2.2%; 95CI 0.05-4.35), Guillain-Barre polyneuropathy – 1 (2.2%; 95CI 0.05-4.35), encephalitis – 2 (4.3 %; 95CI 1.29-7.31), ischemic stroke (AVCI) – 1 (2.2%; 95CI 0.054.35) cases; and 20 (43.5%; 95CI 36.19-50.81) cases presented non-specific neurological manifestations: asthenia – 11 (23.91%; 95CI 17.62-30.2) cases, headache – 5 (10, 9%; 95CI 6.31-15.49), myalgia -– 2 (4.3%; 95CI 1.29-7.31) and anosmia/ageusia – 2 (4.3%; 95CI 1.29-7 ,31) cases. Conclusions. Although most studies have demonstrated the predominance of non-specific neurological manifestations, the results obtained in this study tell us that children with SARS-CoV-2 infection are at risk of developing specific neurological manifestations such as toxic-infectious encephalopathy, seizures and in unique cases neurological complications severe, such as: venous sinus thrombosis, Guillain-Barre polyneuropathy, encephalitis, stroke.
