Juvenile idiopathic arthritis (JIA) is the most common systemic disease causing uveitis in childhood, with a prevalence of 10 per 100 000. Uveitis in the oligoarticular JIA is a challenging issue for rheumatologists and ophthal-mologists due to its unique features - mostly asymptomatic onset, a chronic course, and a high incidence of disabling complications. The objective of the study. The assessment of ocular complications spectrum in children with juvenile oligoarthritis. Material and methods. A retrospective study dated from January 2017 to July 2022 was conducted. 19 pa-tients admitted to Rheumatology Unit, with the age ≤16 years and a diagnosis of juvenile idiopathic arthritis, oligoar-ticular form with eye involvement were included in the study. Results. Demographic analysis of the subjects revealed an average age of 7.05±3.27 years; the average age of onset of oligoarticular JIA was 5.15±2.89 years; onset of uveitis was at the age of 5.82±2.83 years. It was established that the development of juvenile oligoarthritis associated uveitis is more common in girls 84.21% (n=16). Positive titers of antinuclear antibodies were detected in 26.31% (n=5) of cases. The most common form of JIA associated uveitis was chronic anterior uveitis - in 78.95% (n=15) of cases. Asymptomatic onset was present in 73.68% (n=14) of cases. Panuveitis was observed in 15.79% (n=3) and posterior uveitis in 5.26% (n=1) of cases. At least one ocular complication was reported in 73.68% (n=14) of patients. The most common of them were synechiae 63.16% (n=12) and cataract 47.36% (n=9), followed by glaucoma 15.79% (n=3), retinal detachment 15.79% (n=3), band keratopathy 10.53% (n=2), macular edema 5.26% (n=1), corneal dystrophy 5.26% (n=1), and vitreous fibrosis 5.26% (n=1). Bilateral blindness developed in 5.26% (n=1) and unilateral blindness in 15.79% (n=3) of cases. In 21.05% (n=4) cases an eye surgery was performed. Conclusion. Appropriate follow-up and early interven-tion could have an important impact on both - the long-term complications and its negative effects on the quality of patient’s life. Correlation of the obtained data with the results of international studies might serve as a methodical and informational support in the multidisciplinary management of eye disease secondary to systemic disorders based on juvenile oligoarthritis model. |
