Introduction. Non-selective oral non-steroidal anti-inflammatory drugs (NSAIDs) are recommended in international and national guidelines for the management of osteoarthritis (OA) pain in patients with severe and musculoskeletal pain. Purpose of the paper. Reporting the adverse effects of NSAIDs on the gastrointestinal, cardiovascular, and renal systems. Material and Methods. The current international literature on the toxicity of NSAIDs on the aforementioned systems has been studied (EHR base). Results. NSAIDs exert their effects by inhibiting cyclooxygenase (COX), which contributes to the conversion of arachidonic acid to prostaglandins, thromboxanes and prostacyclines. There are 2 isoforms of COX: COX-1-works physiologically in maintaining renal function, protecting the gastric mucosa and regulating platelet aggregation, COX-2 is inducible by proinflammatory cytokines and growth factors. Gastrointestinal toxicity is evident in all NSAIDs, may be dangerous due to ulcers for elderly patients with OA, NSAIDs increase the risk of developing acute myocardial infarction, heart failure and acute kidney damage. Elderly patients are more likely to have cardiovascular disease, impaired kidney function, an increased risk of cardiovascular, hematological, and renal adverse reactions, and patients with OA with comorbid conditions (high blood pressure, heart failure and diabetes) are at increased risk of developing adverse effects. Conclusions. We can ascertain that the side effects of NSAIDs are characteristic of all patients with OA and can be prevented by concomitant administration of proton pump gastroprotective inhibitors or by replacement of non-selective NSAIDs with selective NSAIDs inhibiting COX-2. |
