Background. According to the WHO, severe acute hepatitis (SAH) is an acute inflammation of the liver with serum transaminases > 500 IU/L of unknown cause among young children, with a risk of acute liver failure and the need for vital liver transplantation. To date, 33 countries have reported 650 cases of HAS of unknown origin. Objective of the study to elucidate the current situation regarding the global epidemiological alert by SAH in children and the WHO recommendations on diagnostic and curative management. Material and methods. The relevant bibliographic sources from the WHO, ESPAGHAN, THE LANCET, PUB MED, MEDSCAPE web sites on the number of reported cases, recommendations on the definition and criteria of the clinical case (probable or Epi-linked), diagnosis and patient management were analyzed. Results. Of the 650 reported cases of HAS, 38 (6%) of children required a liver transplant and 9 (1%) died. According to recent data from the WHO and the European Center for Disease Prevention and Control by 23 May 2022, 75.4% of children were under 5 years of age. Out of 181 children tested for adenovirus, 110 of them (60.8%) tested positive, of which F41 type adenovirus was identified. The positivity rate was the highest in whole blood samples (69.5%). Of the 188 cases tested by PCR on SARS-CoV-2, 12.2% (23) of the children were positive. Hepatotropic viruses A, B, C, D, E were not detected in any case. Co-infection with SARS-CoV-2 and adenovirus has been detected in some children. Conclusion. The main concern in the management of the patient with probable case of HAS is to determine the real cause of the disease, estimate the presence of liver failure and clinical indications for liver transplantation. These measures will ensure efficient management and refine control, prevention and treatment actions. |
