Background. About 40 % of juvenile idiopathic arthritis (JIA) patients may not tolerate methotrexate (MTX), and as early it is detected through validated scores, as better the disease activity is controlled. Objective of the study. This study was aimed to find the rate of MTX intolerance through the Methotrexate Intolerance Severity Score (MISS). Material and Methods. A 68 JIA sample case-control study was performed in the Rheumatology Clinic of the Mother and Child Institute. These children and their parents answered the MISS, considered important with intolerance score ≥ 6 points. Demographic data and disease activity were collected from the medical records. Results. The gender distribution was predominantly females 56.3% and the adolescent rate was 54.7%. The disease duration was estimated 5.6±1.1 years, and the duration of MTX administration was 3.2±2.1 years. A MISS of ≥ 6 points was found in 34% of children. The symptoms that established this important rate after MTX use were nausea 86%, abdominal pain 72.8%, vomiting 36% and finally 5% for behavioral symptoms. However, it was found a 94% rate of behavioral symptoms before MTX administration which might be a reason for the patient noncompliance on MTX (p = 0.002). Conclusion. The use of MISS is important in appreciation of intolerance to MTX especially in the frequency of behavioral symptoms that might induce it. The study showed that MISS is a clinical tool for early detection of MTX nonresponse patients. |
