Background: Autoimmune hepatitis (AIH) has a progressive course to cirrhosis and without treatment, most patients die within 10 years of disease onset. An overlap of autoimmune hepatitis and primary biliary cholangitis (PBC) or primary sclerosing cholangitis (PSC) has been well documented. Though, the overlap of autoimmune diseases, other than PBC or PSC, has not yet been investigated. Aims: To study frequency of specific antibodies and the incidence of concurrent autoimmune diseases in patients with AIH. Materials and methods: In this retrospective study were included 31 patients with AIH: 29 women, 2 men, mean age is 45±1.7 years. The diagnosis was confirmed by clinical, biological, histological investigations and presents of serum autoantibodies - antinuclear ANA), smooth muscle (SMA), to liver/kidney microsome type 1 (LKM1) and antimitochondrial (AMA). We performed the level of ALT, AST, GTP, alkaline phosphates, immunoglobulins and, also evaluated the association of other autoimmune diseases. Results: AIH was established more frequently in young age women. The most commonly associated disease has been autoimmune thyroiditis – 29 % (9). It was noted prevalence of hyperaminotransferasemia, ALT level was detected 5.7 times higher than the upper limit of the norm, and AST value - 6.7-fold higher versus ULN. Investigating different types of autoantibodies in patients with AIH, has been revealed the presence of ANA - in 20(64,5%) patients, SMA single - 6(19,4%), anti-LKM1 was detected in 5(16,1%) patients. Thus, according to the obtained results, HAI-type 1 (ANA, SMA positive) was identified in 26(84%) patients and HAI-type 2 (LKM1 positive) in 5(16%). In 5(16,1%) patients were detected cholestatic syndrome and the presence of AMA antibodies. Conclusions: AIH occurs more often in women and was associated more frequently with autoimmune thyroiditis. It has been predominantly detected type I autoimmune hepatitis associated with the presence of ANA antibodies. In some patients were detected cholestatic syndrome and the presence of AMA antibodies, suggesting overlap syndrome with primary biliary cholangitis. |
