Background. Premature birth (PB) continues to be a challenge for contemporary obstetrics and perinatal medicine. The incidence of PB in the world varies between 2.5% and 30%, depending on the single or multiple pregnancy, spontaneous or obtained through IVF, the pre-existing pathology of the pregnancy, and the economic status of the country. The incidence of PB in the R. of Moldova is 5.6%. Objective of the study. Analysis of two clinical cases of extreme PB (EPB) at 24-25 weeks , with triplex pregnancy, obtained by IVF. Material and Methods. Study of medical records of 2 cases of extreme premature birth with triplex. Results. Clinical case 1: Patient 38 years old, with complicated reproductive history, primary infertility, treated conservative 10 years, who had 3 IVF attempts (IVF-failed, IVF 2 performed over 6 months- duplex, finished by EPB at 22 weeks, preterm premature rupture of membranes(PPROM), IVF 3- present pregnancy) Pregnancy 3, within 24+3 weeks, due to PPROM , ended with the birth of 3 newborns, 2 died in the first 7 days , the 3rd is live. Clinical case 2: patient 30 years, primary infertility-3 years, IVF attempt 1, pregnancy finished by caesarean section within 25 weeks , due to premature abruption of the placenta, with negative reproductive result, all newborns were born alive , but died in the first 7 days of life. Conclusion. The cause of perinatal mortality in multiple pregnancies depends on the pathology that triggered the birth and term of pregnancy. |
