Background. Repeated cesarean section presents a major risk factor for hysterectomies caused by pathological placental invasion and atypical localization of the placenta. The risk of placenta accreta / increta / percreta increases with the number of cesarean sections. Objective of the study. Presentation of the rare clinical case of severe complication of the invasion and pathological localization of the placenta at a multiparous at the fifth (5th) caesarean section. Material and Methods. Anamnestic, clinical and paraclinical data were taken from the medical record. The literature on similar cases has been studied. Results. The patient, 40 years old, with 4 cesarean operations in the anamnesis, hospitalized urgently during pregnancy 34-35 weeks with vaginal hemorrhage. At ultrasound examinations during the present pregnancy - the central placenta praevia. Preoperatively ultrasound suspected the placenta increta. Urgent cesarean section in the lower segment, a premature live fetus, 2690 g, 7/8 points Apgar score. Intraoperative pronounced adhesion process, pathological invasion of the placenta with localization at the level of the scar on the uterus with extension on the internal cervical orifice. Total hysterectomy. Hemorrhage - 2500 ml. Hemotransfusion. Histopathological conclusion – Placenta increta Conclusion. The scarred uterus after multiple cesarean operations involves an increased incidence of pathological localization of the placenta. |
