Introducere. Periodontitele cronice reprezintă una din cele mai grave probleme în cadrul endodonției, fiind afecțiuni care duc frecvent la extracții dentare. Tratamentul se bazează pe îndepărtarea bacteriilor din canalele radiculare și vindecării țesuturilor periapicale. Scopul lucrării. Studiul aspectelor legate de diagnosticul și tratamentul periodontitelor cronice, necesitatea alegerii metodei corecte de tratament în dependență de etiologia, forma și tabloul clinic. Material și metode. Studiul a fost realizat pe un eșantion de 11 pacienți diagnosticați cu periodontită cronică. Metoda utilizată pentru 3 pacienți a fost de tratament într-o etapă, ce a presupus prepararea canalelor radiculare și obturarea lor permanentă. Pentru 8 pacienții din a doua grupă de studiu, s-a efectuat tratamentul în 2 etape cu plasarea obturației endocanalare temporare pe bază de hidroxid de calciu între vizite. Rezultate. În rezultat s-a observat că datorită evoluției asimptomatice îndelungate și diagnosticarea tardivă, metoda mai des folosită este cea de tratament în mai multe vizite (73%). La respectarea indicațiilor și urmarea corectă a protocolului rezultatele vindecării periapicale nu prezintă diferențe semnificative în ambele metode. Concluzii. (1) Studiul a demonstrat că reușita terapeutică poate fi atinsă atât prin tratamentul într-o vizită cât și tratamentul în mai multe vizite, dar depinde de abordul individual în fiecare caz și alegerea metodei în dependență de mărimea leziunii, complexitatea sistemului endodontic și de starea generală a pacientului. (2) Succesul tratamentului a fost dovedit prin procesele regenerative survenite, iar ulterior demonstrate prin metode clinice și radiologice.
Introduction. Chronic periodontitis is one of the most serious problem in endodontics, being diseases that often lead to dental extractions. The treatment is based on the removal of bacteria from the root canals and cure of the periapical tissues. Objective of the study. To study aspects related to the diagnosis and treatment of chronic periodontitis, the necessity of choosing the right method of treatment depending on etiology, clinical form and clinical picture. Material and methods. The study was conducted on a sample of 11 patients diagnosed with chronic periodontitis. The method used for 3 patients was treatment in a single visit that involved the pre-paration of root canals and then permanent root filling. For 8 patients from the second study group, multi-visit treatment was performed with placement of temporary calcium-based endocannal medication between visits. Results. As a result, due to asymptomatic long-term development and late diagnosis of disease, the more commonly used method is multi-visit treatment (73%). When respecting the indications and correctly following the protocol, the results of the periapical healing show no significant difference in both methods. Conclusions. (1) The study demonstrated that therapeutic success can be achieved both by treatment in a single visit and in multiple visits, but it depends on the individual approach in each case and the choice of method based on the size of the lesion, the complexity of the endodontic system and the patient’s condition. (2) The success of treatment has been proven through regenerative processes, and subsequently demonstrated by clinical and radiological methods.
