Background. Acute posterior multifocal placoid pigment epitheliopathy (APMPPE) is a rare inflammatory disease. It affects young individuals causing decreased vision, photopsia, central scotomas, which according to several observations, may be preceded by an autoimmune disease such as erythema nodosum. Objective of the study. To describe a clinical case of a young woman with an acute binocular visual decrease caused by APMPPE preceded by erythema nodosum. Material and Methods. A full ocular examination including multimodal imaging. Results. A 20 years woman complained of decreased vision and photopsia in both eyes, 1 week following the onset of erythema nodosum. On examination, best corrected visual acuity (BCVA) Right eye 0.6, Left eye 0.7. Anterior segment within normal limits. Funduscopy showed multiple yellow-white placoid lesions in the macula, confluenced between them. Spectral domain optical coherence tomography revealed typical lesions in outer retinal layers. Due to the macular involvement and decreased vision it was administered retrobulbar corticosteroid injection with Kenalog 1 ml in both eyes. The recovery of visual function was noticed after 2 weeks following the corticosteroid therapy. Conclusion. Erythema nodosum can cause APMPPE with a favorable evolution and fully recovery of visual fonction. |
