Background. Acute stroke is ranked as the second leading cause of death in the Republic of Moldova. 13% of patients are of working age and a third of them are socially active people. The incidence of stroke is 2,5-3,0 of 1000 per year. About 35% of patients die during the acute phase of the disease. Objective of the study. Research of the risk factors for unfavorable prognosis in patients with acute stroke. Material and Methods. Retrospective study based on 85 cases of death in patients with stroke. The main risk factors leading to an unfavorable prognosis in patients with acute stroke were analysed: age; the first signs of the disease; the time when the 112 emergency service was requested; comorbidities; clinical examinations performed in the Emergency Department (ED). MedCalc Statistical Software was used. Results. Risk factors for unfavorable prognosis in patients with acute stroke are: life in urban areas 77%; age: for ischemic stroke - 70-79 years, for hemorrhagic stroke – 50-59 years; patients’status at hospitalization: coma - 33,7%, dizziness - 22,9%; time from hospital arrival to initiation of thrombolytic therapy > 6 hours - 34%, 4-6 hours - 23%, 3-4.5 hours - 23%; first-time stroke - 66%; left ventricular hypertrophy electrocardiographically detected - 96%. It has been found that obesity is not a risk factor for unfavorable evolution of the disease in 92% cases. Hyperglycemia in patients without a history of diabetes - 73 % and pressure values ≤ 180/100 mmHg - 89% are risk factors. Conclusion. Risk factors for unfavorable prognosis in patients with acute stroke are: low level of patients’knowledge concerning the first signs of the disease and immediate call to 112 emergency service, which are important for rapid transportation of patients to the ED and prompt treatment initiation. |
