Background. Brugada’s Syndrome (BS) is a genetic disorder what is characterized by malignant ventricular arrhythmias, abnormal electrocardiographic signs, making up 4% from all cases of sudden deaths while 20% are in pacients with an structurally intact heart. Objective of the study. Evaluation of electrocardiographic changes in Brugada’s Syndrome. Material and Methods. Analysis of cardiologic literature with selection of electrocardiographic changes in pacients with Brugada’s Syndrome for making an early diagnosis and the correct choice of the treatment Results. Brugada’s Syndrome is an autosomal dominant genetic disorder which has an evolution with: sincopa and nocturne agonal respiration caused by ventricular arrhythmias. Electrocardiographically in leads V1-V3 are found: ST descending oblique elevation followed by a negative T-wave – type 1 „coved”, ST elevation followed by a positive T-wave “ – type 2 „saddle-back”. Due to the prevalent implication of the right ventricle (RV) the BS was considered a RV cardiomyopathy with undetectable structural changes in diagnostic investigations. Treatment of arrhythmias: potassium channel blockers, betamimetics,phosphodiesterase-III inhibitors and cardiac defibrillator implant Conclusion. Ventricular arrhythmic storms in an asymptomatic pacient with changes on ECG: as ST elevation, positive or negative T-waves in leads V1-V3 requires a suspicion of Brugada’s Syndrome with implantation of cardiac defibrillator for prevention of sudden cardiac death. |
