Background. Polytrauma meets the classification criteria for a global pandemic and it is a significant cause of mortality and morbidity despite global efforts to control its effects. Objective of the study. The purpose of this study is improving the diagnosis, specifying the indications and time of the intervention, selecting the surgical approach, the optimal fixators with the specification of functional recovery tactics in polytrauma patients. Material and Methods. Patient X aged 52 years old, after receiving a trauma by falling from a height of 5m, was transported urgently to the hospital of the DMU the pwas still conscious, time-oriented and space-oriented, in severe but stable condition, hemodynamically stable, and the lower leg as immobilized on the splint. The multidisciplinary team according to the principles ATLS. Results. The diagnosis has been established: Polytrauma. Catatrauma. Bilateral slight pulmonary concussion. Tile pelvic fracture type-C. Subtrochanteric closed fracture of the proximal femur to the right of Seinsheimer Type – IV (AO – 31A3). Multiple body scoring. ISS-23 points. Clinically and radiologically investigated by the multidisciplinary team.Thus, the decision is made to be hospitalized for urgently postponed surgical treatment. In 5 days, 3 interventions were performed in one stage. After which the patient's condition is of stable average severity. the patient was discharged more than 10 days after admission,without any complications. Conclusion. The patients with polytrauma,with an ISS score of between 17-25 p. with predominant extremity and pelvic ring and fixed hemodynamic,can be urgently treated surgically. This will result in a low hospital rate, minimal postoperative complications and faster patient recovery. |
