Objective of the research-to evaluate violations of intrarenal hemodynamics in children with vesicoureteral reflux (VUR), depending on sclerotic kidney disease severity. Study materials and methods: the study included 79 children with VUR complicated with urinary tract infection (UTI) aged 1-17 years (mean age 10,2±4,5 years), incl. 57 (72,2%) girls. All patients underwent complete nephrourologic examination using a set of anamnestic, clinical, laboratory and instrumental methods. Doppler study was performed using color Doppler mapping and pulse Doppler. Results: the article presents results of renal hemodynamics research at the level of interlobar and interlobular arteries, since we did not find a reliable difference between all levels of arteries. The study revealed a clear dependence of IR disorders incidence in interlobar and interlobular arteries on the VUS degree, and the decrease in IR indices is typical only for VUS high degree. It also proved that IR indices at the level of interlobar and interlobular arteries depend on refluxnephropathy (RN) degree and are not associated with the lesion side. Conclusion: an increase in RN degree is accompanied with low IR frequency increase (<0,6) and a decrease in number of children with normal IR at the level of interlobar and interlobular arteries. At III-IV RN degree in 29,2% of cases, the blood flow velocity was decreased and almost in half of the patients (47,9%) was increased. In children without RN IR disorders in interlobar and interlobular arteries are rarely detected and in the form of an elevated IR index. |
