Rezumat

Objective: Metabolic syndrome (MetS) and arterial systemic hypertension represent important cardiovascular risk factors that determine patient’s prognosis. Their impact on global function of right ventricle (RV) is not completely clarifi ed. The aim of our study was to assess the infl uence of blood pressure (BP) values and other components of MetS on global (systolic and diastolic) RV function, determined by Tei index. Design and method: Our study included 68 subjects (31 males and 37 females, mean age 55.28 ± 7.28 years) with MetS. MetS was defi ned by the presence of 3 or more criteria of International Diabetes Federation and American Heart Association/National Heart, Lung, and Blood Institute. All subjects underwent careful clinical and physical examination, laboratory blood tests, complete twodimensional, pulsed and tissue Doppler echocardiography. Tei index of RV was determined by pulsed and pulsed tissue Doppler. Results: The mean systolic BP (SBP) was 143 ± 11 mmHg, the mean diastolic BP (DBP) was 93 ± 10 mmHg, the mean value of plasma glucose (GLU) level was 5.45 ± 1.21 mmol/l, the mean value of HDL-cholesterol (HDL) level was 1.14 ± 0.36 mmol/l, the mean value of triglycerides (TG) level was 1.86 ± 0.38 mmol/l, the mean values of waist circumference (WC) was 95 ± 12 cm in women and 107 ± 11 cm. The mean values of Tei index of RV determined by pulsed and pulsed tissue Doppler were, respectively 0.46 ± 0.03 and 0.57 ± 0.03. The Tei index of RV determined by pulsed tissue Doppler was positively correlated with SBP (r = 0.780), DBP (r = 0.692), GLU (r = 0.325), TG (r = 0.155), WC (r = 0.520) and negatively related to HDL (r = –0.007). Multivariate analysis revealed that SBP (ß = 0.102, p = 0.035) and WC (ß = 0.113, p = 0.025) were independently associated with RV Tei index determined by pulsed tissue Doppler. Similar data were obtained for the Tei index determined by pulsed Doppler. Conclusions: MetS plays an important role in damage of global RV function, measured with help of Tei index. Among all the components of MetS, SBP and WC were independently associated with RV Tei index.
