An unusual case in dental practice
The article discusses an extraordinary case in dental practice.
A 49-year-old woman visited the dentist complaining of chronic severe pains around tooth 1.8, these being diffuse in character and accompanied by impaired respiration on the right part of the nose. On paraclinical examination the diagnosis of exacerbating chronic odontogenic sinusitis involving tooth 1.8 has been established; Radicular suppurating cyst in tooth 1.8. The anamnesis data showed the patient to have been treated by a neurologist for acute diffuse neuromyositis of the cervical muscles in the right shoulder blade. The administered treatment showed no improvement.
On the first visit to the dentist, tooth 1.8 was extracted and the cystectomy performed being accompanied by purulent discharge. The alveolus and the communicating maxillary sinus were treated with 3% Sol. H2O2, furacilin. A tent was introduced into the alveolus; antibiotics and antifungal drugs were adminstered. The next day the patient’s condition improved: facial pains and neurological symptoms decreased considerably.
On the 8th day the patient was pain-free. The case being discussed with neurologists, the diffuse pains were explained by the anastomosis between nerves from C1, C2 and the trigeminal, they radiating to the head, shoulder, etc. Removing the odontogenic factor and sinusitis lead to complete recovery. |
