Introduction. Thumb sucking is an abnormal habit that occurs in childhood and can cause several malocclusions if it persists for a long time. Malocclusions caused by oral habits require proper treatment timing to maintain a normal growth and should be treated at an early age. This case report shows the management of thumb sucking and early correction of anterior open bite by a modified Haas expander. Three-year follow-up results showed the effectiveness of this special designed appliance. Objective of the study. Presentation of clinical case of a patient with openbite treated by RME. Material and Methods. Anamnesis, clinical and paraclinical data were taken from the medical databases. The patient was taken impressions from upper and lower jaws. The literature about similar cases has been studied. Results. Thumb sucking was reduced significantly in the first weeks with the appliance and was prevented completely at the end of the first month. The delayed permanent anterior teeth eruption was normalized when finger pressure was eliminated. The appliance was removed at the end of 6 months and was recalled for follow-up visits at 3-month intervals for 3 years. Conclusion. The long-term stability of open bite correction associated with thumb sucking in the mixed dentition was favorable. The unique design of the appliance provided an opportunity to resolve a major problem with one appliance and can be preferred as a convenient alternative to conventional habit breakers.

Introducere. Obiceiul vicios de interpoziție cu sucțiune intensă (sugerea degetului) ce apare în copilărie provoacă tulburări de creștere în sectorul frontal. Acesta nefiind tratat la timp poate duce la diferite malocluzii. Cazul clinic prezentat ne demonstrează tratamentul efectiv al ocluziei deschise cauzate de sugerea degetului cu ajutorul disjunctorului (maxilar) palatinal Haas. Scopul lucrării. Evaluarea metodei de tratament ortodontic al compresiunii de maxilar superior asociată cu ocluzie deschisă cauzată de un obicei vicios (sugerea degetului)prin utilizarea disjunctorului (maxilar) palatinal. Materiale și metode. Pacienta M.B, 8 ani, s-a adresat cu acuze estetice și funcționale, iar în urma examinării clinice și paraclinice s-a stabilit diagnosticul de ocluzie deschisă. S-a efectuat examenul clinic intra- și extraoral, examenul fotografic, amprentarea arcadelor dentare, studiul pe model și examenul radiologic. Rezultate. Tratamentul ortodontic a urmat cu succes prin reducerea obiceiului vicios cu ajutorul disjunctorului timp de 1-2 luni. Aparatul s-a scos după un interval de 6 luni și pacientul a fost chemat la control din 3 în 3 luni. Concluzii. Ocluzia deschisă, ce se prezintă clinic prin lipsa de contact inter-dentar în plan vertical, poate fi tratată cu succes prin intermediul tratamentului ortodontic cu ajutorul disjunctorului fix maxilar Haas.
