Background. Distribution errors are a real public health problem and lead not only to changes in the expected therapeutic results, but also to an increase in morbidity and mortality rates, as well as in the costs associated with healthcare. Objective of the study. Identification of the causes that generate the occurrence of distribution errors and their impact on the pharmacist’s activity. Material and Methods. An approach with mixed methods was adopted, incorporating prospective disguised observation of distribution errors and interviews with pharmacists on the causes of errors. A survey divided into the following compartments was applied: socio-demographic factors, professional status, workplace, work task, psychological state of the pharmacist. Results. The total rate of drug dispensing errors was 24.6%, of which 11.5% were prescription errors and 13.1% patient counseling errors. The most common type of prescription-related errors was the wrong amount (37.9%), while the most common error in counseling patients was the wrong drug (41.9%). Most of the errors were caused by illegible writing of the name of the drug (30.7%), followed by high workload (17.3%). More than half of the errors (52.6%) were moderate in severity, followed by minor errors (38.8%), and 8.6% of errors were assessed as serious. Conclusions. Educating and training pharmaceutical staff on how to properly manage medication is a fundamental element of the process of improving the safety of health services. Pharmacists must first know the causes that can lead to distribution errors, and then have at their disposal tools to help them in the process of dispensing medicines. |
