Background: The incidence of true umbilical cord (UC) nodes varies from 0.3% to 1.2%, and of multiple true nodes even less often, and the mortality of the domain 10%. The use of three-dimensional USG has improved the accuracy of prenatal diagnosis by up to 62.5%, thus reducing stillbirth. Aim of the study. Assessment of pregnancy and birth behavior in the presence of a triple umbilical cord node confirmed by USG. Material and methods. Assessment of the pregnant woman and the analysis of the medical file of a complicated case of birth with a triple umbilical cord node. Results. Clinical case. The patient, aged 31 years, G3 N3, addressed for a scheduled pregnancy monitoring consultation for the USG exam at the term of pregnancy 36-37 weeks. Data on intrauterine distress of the fetus were found at EUSG: reduced velocimetric indices on MCA, and a loop-like structure was visualized, a UC node fetal, polyhidramnios. The diagnosis of progressive fetal hypoxia is established (suspected umbilical cord pathology). It is decided to perform an emergency cesarean section. A male fetus is extracted, weighing 3320g, with a double circular pericervical and single at the level of the foot, the Apgar score in the first min-5 points, and over 5 minutes - 7 points. When examining the cord, there was a real knot, with triple knotting. Conclusion. Patients with risk factors for the true umbilical cord node need to be monitored more closely, and the need to include additional USG tests to rule out complications of antenatal death, or IUGR. |
