Background. Pulmonary arterial hypertension (PAH) is a severe disease that requests modern methods of diagnosis as cardiac catheterization and a timely initiated specific treatment with bosentan and sildenafil contributes to a significant improvement of the health condition. Objective of the study. To present a case report of a child with PAH treated with bosentan and sildenafil with dynamic evaluation of the efficiency criteria. Material and Methods. FC WHO/NYHA, Oxygen saturation, 6MWT (6-min walk test), mPAP (mean pulmonary artery pressure), CI (cardiac index), PVR (pulmonary vascular resistance) were evaluated at the beginning, at 6 and 12 months. Results. A 5 years old child was diagnosed by echocardiography and computer tomography with CHD, PAD (7 mm length and 9×7mm in diameter) and severe PAH (mPAP - 70 mm/Hg). According to existing cardiac catheterization data PVR/SVR res ratio - 1.03, the surgery is questionable, and pharmacological treatment is initiated with sildenafil 10mg 3 times/day and bosentan 16 mg 2 times/day. Initially, CF FC III NYHA/OMS, mPAP – 70 mm/Hg, oxygen saturation - 97/68%, CI - 3,55, 6MWT - 320 m, after 6 months CF FC II NYHA/OMS, mPAP – 43 mmHg, oxygen saturation - 97/80%, CI - 2,64, 6MWT – 490 m and after 1 year of treatment CF FC I-II NYHA/OMS, mPAP - 37mm/Hg, oxygen saturation - 98/87%, CI - 2,5, 6MWT 550m. Conclusions. The combined and specific treatment with bosentan and sildenafil initiated in children with PAH reveals a significant positive dynamic of the efficiency criteria. |
