Rezumat

Introduction: Postpartum disorders are a separate problem in mental disorders. In some cases shortterm psychotherapeutic interventions are sufficient, and in others pharmacological intervention is required. Materials and methods: The paper presents a literature review of more than 500 scientific articles. Search engines used are PubMed, Medscape, www.nice.org.uk, www.ncbi.nlm.nih.gov. Results: Psychotherapy is recommended for mild to moderate PPD. If the condition has been not improved, antidepressant drugs are indicated - SSRIs, where the Relative Infant Dose (RID) does not exceed 10% of the maternal dose received. Could be used SNRIs, neurosteroids as brexanolone, approved by FDA in 2019, estrogens, thymostabilizers, tricyclic antidepressants (TCAs ), atypical antipsychotics with a low RID value. Conclusions: The risks and benefits of the each drug will be discussed with the beneficiary. Most of the side effects occur due to transmission of the drug trhough breast milk. For these reasons will be selected the drug with the lowest risk which will be used at the lowest dose. Monotherapy will be preferred. The adverse effect of the drug will be minimal if the mother takes it immediately after breastfeeding. In some situations, the plasma concentration of the drug or metabolites will be measured.
