Introducere: Morbiditatea în apendicita acută la bătrâni este determinată de adresabilitatea tardivă şi predominanţa formelor distructive. Scopul: Perfectarea diagnosticului apendicitei acute la bătrâni prin implementarea scorului Alvarado. Material şi metode: Studiul retrospectiv vizează 224 pacienţi vârstnici cu apendicită acută, trataţi în perioada 2013-2018. Bărbaţi – 88 (39%), femei – 136 (61%). De vârstă 60-74 (youngold) au fost 179 (79.9%), 75-84 (middle-old) – 41 (18.3%), ≥ 85 (old-old) – 4 (17.7%). S-au analizat semnele clinice şi formele morfopatologice ale apendicitei acute (AA) în corelaţie cu criteriile scorului Alvarado. Rezultate: Scorul Alvarado: Semnul Kocher – 141 (62.94%), inapetenţa – 139 (62.05%), vome (greţuri) – 108 (48.21%), dureri (sensibilitate) locale – 223 (99.5%), semnul Blumberg – 164 (73.21%), febra (> 37,3°C) – 122 (54.46%), leucocitoza (> 10.0×109) – 101 (45.08%), devierea formulei leucocitare (> 75%) – 81 (36.16%). Morfologic s-a depistat: apendicită gangrenoasă la 100 (44.64%), flegmonoasă – la 32 (14.28%), catarală – la 92 (41.08%). Scorul Alvarado: (< 5 puncte) AA îndoielnică – 35 (15.62%), AA posibilă (5-6 puncte) – 91 (40.6%), AA probabilă (7-8 puncte) – 69 (30.8%), AA foarte probabilă (> 8 puncte) – 29 (12.94%). Concluzii: Scorul Alvarado izolat la bătrâni este informativ în stabilirea diagnosticului. Formele distructive ale apendicitei acute sunt prognosticate prin calcularea scorului Alvarado în mai puţin de jumătate de cazuri. Lipseşte interacţiunea direct proporţională între forma morfologică şi valoarea scorului. Se determină cert tendinţa creşterii incidenţei formelor distructive cu avansarea în vârstă.

Purpose: Implementing the Alvarado score in the diagnosis of acute appendicitis in the elderly patients. Material and methods: Retrospective study contains 224 elderly patients with acute appendicitis, during the period 2013-2018. Men were 88 (39%), women – 136 (61%). Aged 60-74 (young-old) – 179 (79.9%), 75-84 (middle-old) – 41 (18.3%), 85 – 4 (17.7%). Alvarado score: Kocher sign – 141 (62.94%), inadequacy – 139 (62.05%), vomiting (nausea) – 108 (48.21%), local pain (sensitivity) – 223 (99.5%), Blumberg sign – 164 (73.21%), fever (> 37,3°C) – 122 (54.46%), leucocytosis (> 10.0×109) – 101 (45.08%), leukocyte left shift (> 75%) – 81 (36.16%). Results: Morphologically it was established: gangrenous appendicitis in 100(44.64%), phlegmonous appendicitis in 32 (14.28%) and catarrhal appendicitis in 92 (41.08%). Alvarado score: (< 5 points) doubtful AA – 35 (15.62%), AA probable (5-6 points) – 91 (40.6%), AA probability (7-8 points) – 69 (30.8%), very probable AA (> 8 points) – 29 (12.94%). Conclusions: The Alvarado isolated score in the elderly is informative in establishing the diagnosis. The destructive forms of acute appendicitis are diagnosed by calculating the Alvarado score in less than half of the cases. There is no direct proportional interaction between morphological forms and Alvarado scores. The tendency to increase the incidence of destructive forms with the aging of the elderly is determined.
