Background. Recent studies have suggested that ischemic stroke in young adults is increasing and have demonstrated that risk factors are different from those common among older adults. Reduction of risk factors represents an important key in the decreasing of morbidity and mortality. Objective of the study. Comparative analysis of young patients with stroke stratified by the presence of large vessel atherosclerosis. Material and Methods. A retrospective analysis of all medical records of patients with ischemic stroke from „Diomid Gherman” INN during 2018- 2019 (1687 patients) was performed, 56 patients aged 50 and less were selected from which 44 presented ultrasonographic evaluation of large vessels and were stratified in Gr. I = 18 patients with atherosclerosis and Gr II = 26 without atherosclerosis (31 man and 11 women). Results. The mean age of patients was 42.95±6.7 years., NIHSS at admission 10.03±5.14p. Impaired consciousness at stroke onset in Gr I - 66.7% vs Gr. II - 33.3%. Neurologic deficit at stroke onset in Gr. I - 33.7% vs Gr. II - 66.7 %. Speech disturbances at stroke onset in Gr. I was present in 45.5% cases and in Gr. II 54.5%. Speech disturbances were present in 60.0% Gr. I vs 40.0% in Gr. II. Sensibility disturbances in 66.7% cases Gr. I and in 33.3% patients Gr. II. From 31 patients with paresis/ plegia, 10 patients (32.3%) had atherosclerosis and 21pacients (67.7%)- have not. 14 patients have manifested bulbar syndrome- 42.9% (6p) with atherosclerosis and 57.1% (8p) without. Conclusion. In young ischemic stroke patients with atherosclerosis, impaired consciousness was the most frequent onset of stroke. Clinically, they mostly presented speech and sensibility disturbances, compared to patients without atherosclerosis that presented paresis/ plegia and bulbar syndrome. |
