Background. In clinical protocols and international guidelines, beta-adrenoblockers have been a leading group in the treatment of cardiovascular disease. Their argument was based on the pharmacological effects and pathogenesis of hypertension, ischemic heart disease, heart failure. Objective of the study. Study of the peculiarities of metabolism of metoprolol, bisoprolol, carvedilol, nebivolol when selecting in the treatment of these diseases, especially depending on the functional state of the liver. Material and Methods. Selected data from Pubmed, Cochrane databases on beta-adrenoblockers metabolism were analyzed, including with the participation of hepatic microsomal enzymes. National clinical protocols and guidelines for international cardiology associations regarding recommended beta-adrenoblocks were studied. Results. The systematization of data on the metabolism of beta-adrenoblockers, including the genetic polymorphism of cytochrome P-450 isoenzymes, estimated the peculiarities of the metabolism of those drugs and the development of selection principles in patients with cardiovascular disease and liver disease. |
