Background. Purulent diseases in puerperal period obviously increase the rate of maternal morbidity and mortality. According to the literature 1 out of 5 women with cesarean section are complicated in the postpartum period with puerperal infections. Objective of the study. Analysis and highlighting of factors associated with the high rate of puerperal complications after cesarean section compared with vaginal births. Material and Methods. It is a retrospective study on a group of 609 patients with puerperal infection who were hospitalized in Clinical Hospital nr. 1 in the Septic gynecology department during 2016-2018years that was divided into group I (405 patients), with complications after cesarean section and group II (204 patients) - after vaginal delivery with puerperal infection. Results. The study was carried out on 609 patients, with extremes of 18 and 46 years respectively, the average age being 32 years. According to the results obtained, the septic-purulent complications were more frequent in the first group - endometritis (33.4% cases, versus 66,5% cases, p<0,01) methroendometritis (10% cases versus 4% cases, p<0,01), paravezical infected hematoma - (0.5% cases versus 0 cases), abscess with intra-abdominal fistula - (1% of cases versus 0% cases), suppuration and dehiscence of the wound in the uterus in the group I - 7.7%, dehiscence of sutures of perineal laceration in the group II - 11% of cases, (p<0.01). Conclusion. The predisposing factors for puerperal infections are: complicated obstetrical history, inflammatory extragenital pathology, prolonged labor and the presence of hematoma after delivery. |
