Background. Assessing the severity of the injuries, as well as estimating the polytrauma patient’s prognosis are the key moments in their management. Predictive models for polytrauma treatment outcome are poorly described in the literature. Objective of the study. Polytrauma treatment outcome predictive model elaboration for elucidating the pathophysiological mechanisms, prophylaxis and potential treatments of this nosology. Material and Methods. In the clinical study (67 polytrauma patients), proteases, antiproteases and treatment outcome (survival/death) were considered. They were used three statistical instruments - dimension reduction, correlational analysis and multivariate logistic regression. Results. Initially, to reduce the dimension, the factorial analysis was applied followed by correlation analysis to highlight the potential predictors for outcome. Four predictive models were developed. The comparative evaluation showed superiority of the model at 24 hours after trauma (calibration χ²=1.547, df=7, р=.981; determination, 0.759; discrimination, sensitivity 90.7 %, specificity 81.8 %, area under RОС curve 0.95 (95%CI 0.912, 1.000)). The model enrolled four latent factors, gender and ARDS. Conclusion. In this study, four predictive models for outcome in polytrauma were developed, predictors being the latent factors from dimension reduction analysis. These allow the elucidation the pathophysiological mechanisms, the prophylaxis and the potential treatments of this nosology. |
