Background. Autoimmune hypothyroidism is one of the most common organ-specific autoimmune disorders. There are several correlations between autoimmune hypothyroidism and subclinical inflammation, but most studies do not evaluate the change in inflammatory biomarkers during treatment. Objective of the study. To evaluate the effect of levothyroxine therapy on biomarkers of subclinical inflammation in patients with hypothyroidism. Material and Methods. The study included 19 patients with recently diagnosed hypothyroidism, who were not treated with hormone replacement therapy. 11 patients were investigated after 2 and 4 months of treatment with levothyroxine with an average dose of 0.7-1.6 mcg / kg / day. The investigated parameters were TSH, fT4, hs-CRP, ESR, fibrinogen. The t-Student statistical test was performed. Results. Out of 19 patients with elevated TSH, 15 had low fT4 (overt hypothyroidism) and 4 had normal fT4 (subclinical hypothyroidism). All the patients had increased anti- thyroperoxidase and/or anti-thyroglobulin antibodies level. There was a significant decrease in TSH over time (P<0.0001) (initial levels were on average 56,29 μIU / mL and și 2,33 μIU / mL at 4 months). There was a significant increase in FT4 (P < 0.005) (initial levels were on average 0,56 ng / dL and și 1,2 ng / dL at 4 months). A decreasing trend of hs-CRP was observed over time (initial levels were on average 2.29 mg / L, 1.83 mg / L at 2 months and 2.19 mg / L at 4 months, P = 0,23). Conclusion. This study showed a tendency in decrease of hs-CRP in patients with hypothyroidism under treatment. However, the small sample size and the heterogeneous group of patients (both with clinical and subclinical hypothyroidism) impose further research. |
