There is much discussion and controversy regarding the optimal treatment of displaced fractures of the proximal humerus. From the multiple options of osteosynthesis (percutaneous nailing, plates with/without angular stability and interlocking nailing) the Seidel centromedullary nail must be considered as a possible choice for 2 part Neer humeral fractures. Between 2004 and December, 2006 we operated on 12 patients (8 male, 4 female) with locked Seidel nails for fractures of the proximal humerus. 3 of the fractures were caused by politrauma and 2 were associated with fractures of the humeral shaft. The postoperative clinical and radiological survey lasted for about 1 year. All fractures healed within a mean time of 5 weeks. In 3 cases the nails were poorly inserted in the humeral head, resulting in impingement. In 1 case, the locking of the distal nail partially failed due to a very large intramedullary canal. In 2 cases there were temporary problems with heightened sensitivity in the exterior of the shoulder. In 1 case, the proximal locking screw had migrated. Results show centromedullary osteosynthesis with Seidel locked nail is a valuable and less invasive option for the treatment of proximal humeral fractures. |
