Cercetările contemporane presupun că răspunsul imunitar la acţiunea antigenelor endogene (lipoproteidele
cu densitate scăzută) şi extragene (adenoviruşii ş.a.) are o importanţă primordială în apariţia aterosclerozei
arterelor. La etapa primară a aterogenezei în stratul subendotelial al intimei se depun lipoproteidele cu
densitate joasă, care devin astfel de caracter autoantigen şi stimulează formarea moleculelor hemoadezive cu
moleculele endoteliale. Acestea, la rândul lor, se leagă cu liganzii leucocitelor agranulate. Monocitele/macrofagele
şi T-celulele activate, pătrunse în intimă, iniţiază procesul de infl amare a „fazei acute” la etapa primară a
aterogenezei. Participarea celulelor intimei în procesele de infl amare (macrofagele, celulele musculare netede
şi celulele endoteliale produc antigeni de gradul II) transformă procesul acut în unul cronic.
Până nu demult se considera că ateroscleroza este legată numai de dezvoltarea hipercolesterinomiei şi
de depunerea în surplus a lipoproteidelor pe pereţii vaselor sanguine, ceea ce cauzează formarea plăcilor aterosclerotice.
Însă, răspunsul molecular al pereţilor vaselor determină evoluţia procesului, care poate regresa
sau progresa spre formarea plăcilor aterosclerotice. Citokinele, produse din moleculele intimei locale sau de
provenienţă hematogenă, pot avea un rol determinant în procesele patologice din pereţii vasculari, indiferent
de nivelul lipidelor în sânge.

the heart, thus acquiring an auto-antigenous character which stimulates the formation of hemoagdezic and endotelial
molecules. These, in turn, link with the liganses of a-granulated leucocytes. Monocytes/macrophages
and activated T-cells, which entered the heart, initiate the infl ammation process of “acute stage” at the initial stage of atherogenesis. The participation of heart cells in the infl ammation processes (macrophages, smooth
muscles cells and endotelial cells produce antigenous of II degree) transform the acute process into a chronic
one.

and exceeded sediments of lipoproteids on the walls of blood sanguiferous vessels, which cause
the formation of sclerotic plates. However, the molecular response of the vessels walls determine the process
unfolding, which can either regress or progress the formation of atherosclerotic plates. Cytokines, produced
from local heart molecules or from hematogenic, can have a determinant role in the pathologic processes which
unfold on the walls of sanguiferous vessels, despite the level of lipids in the blood.
Данные современных исследований не дают основания утверждать, что иммунный ответ на
эндогенные (мЛПНП) или экзогенные (аденовирусы) антигены могут играть первичную роль в
формировании начальных атеросклеротических поражений артерий. В то же время нет оснований
утверждать что такой процесс не происходит. В начальной стадии атерогенеза отложения и
образование в субэндотелиальном слое интимы мЛПНП, приобретающих аутоантигенные свойства,
является тем пусковым фактором, который стимулирует экспрессию эндотелиальными клетками
хемоадгезивных молекул, связывающихся с лигандами негранулярных лейкоцитов. Активированные
моноциты / макрофаги и Т-клетки, проникающие в интиму, в ранних стадиях атерогенеза запускают
каскад реакций, обеспечивающий развитие «острой фазы» воспаления. Включение клеток интимы в
воспалительные реакции (макрофаги, ГМК и эндотелиальные клетки начинают продуцировать антигены
II класса) переводят острую фазу воспаления в хроническую по типу реакции гиперчувствительности
замедленного типа. Еще недавно считалось, что атеросклероз и развитие гиперхолестеринемии,
отложение в сосудистой стенке липопротеидов, обогащенных холестерином является основным
условием образования атеросклеротических бляшек. Однако, клеточный ответ сосудистой стенки
определяет по какому пути будет развиваться процесс: подвергаться регрессии или прогрессировать в
сторону образования осложненных атеросклеротических бляшек. Цитокины продуцируемые клетками
интимы местного и гематогенного происхождения, могут играть определяющую роль в патологических
процессах, протекающих в сосудистой стенке, независимо от уровня липидов в крови.
