Background. Acquired cardiac disease remains a major cause of morbidity and mortality to pregnant women and postpartum: it complicates 0.2 - 4% of pregnancies, causes 10-25% of maternal deaths. Objective of the study. Pregnancy management to a multiparous patient with cardiac disease, with multiple cardiac defects and atrial fibrillation, detected primarily during the third pregnancy. Material and Methods. Objective clinical examination, evaluation of laboratory and instrumental results, from the medical database of a pregnant woman with acquired cardiac disease. Results. Clinical case: Patient 40 years old, from the obstetrical anamnesis - 2 pregnancies, without cardiovascular diseases, at 23 weeks contacted COVID-19 infection. At 25 weeks, it was complicated by bronchopneumonia and atrial fibrillation - indication for re-hospitalization to the Cardiology Clinic. By a complex paraclinical examination was detected primary cardiac disease, treated in the common council: cardiological-obstetrical, planning pregnancy management. At 36 + 5 weeks woman was hospitalized in Mother and Child Institute. According to the decision of the council, oral anticoagulant was replaced by low molecular weight heparins until 38 weeks, the pregnancy being finished by caesarean section, with a living newborn, Apgar 6/8. Postpartum heparin was replaced by oral anticoagulant under the control of the coagulogram. Conclusion. Pregnancy complicated with cardiac pathology treated by cardiologists and obstetricians, ensures the prophylaxis of severe thromboembolic complications. |
