Introduction. Benign prostatic hyperplasia is a very common pathology, causing lower urinary tract symptoms and sexual dysfunction. The surgical treatment of benign prostatic hyperplasia has become very complex. New endourological methods allow to both effectively treat benign prostatic hyperplasia and maintain sexual functions postoperatively. The objective of the study was to assess the possibilities of maintaining ejaculatory function in patients with benign prostatic hyperplasia undergoing Thulium:YAG laser vapoenucleation of the prostate. Material and method. The study was conducted within the Department of Urology, on a group of 23 patients who underwent Thulium:YAG laser enucleation of the prostate gland, with preservation of paracollicular tissues. Functional performance and sexual function parameters were assessed at 3 and 6 months after surgery. Results. Thulium: YAG laser vapoenucleation of the prostate with preservation of paracollicular tissue has proven to be functionally effective while maintaining the erectile function. The occurrence of retrograde ejaculation decreased to 39.1% when using the modified method of prostate vapoenucleation. Conclusions. The ThuVEP paracollicular tissue-sparing technique allows to maintain erectile function in sexually active patients and offers the opportunity to reduce the retrograde ejaculation occurrence. However, the relatively high incidence of retrograde ejaculation requires improvement and rationalization of surgical techniques aimed at preserving physiological ejaculation. |
