Background: Patients with large benign prostatic hyperplasia (BPH) (> 80 cm3) are of particular interest in selection of the surgical treatment method. The attempt to use laser technologies in large BPH surgery has allowed effective treatment with minimal risks. Objective of the study. Comparative evaluation of the efficacy and safety of transurethral Thulium:YAG laser vapoenucleation of the prostate (ThuVEP) and transvesical adenomectomy (AE) in patients with large BPH. Material and Methods. 59 patients (64 ± 7 years) with large BPH were randomly divided into 2 groups for surgical treatment: ThuVEP - 25 patients and AE - 24 patients which were assesed at 3 months postoperatively. Hemoglobin level drop was assessed at 24 hours postoperatively. Inclusion criteria: prostate volume> 80cm3, IPSS> 20, QoL> 4, PVR> 50 ml. Results. Significant difference in mean operative duration was determined: 93±7 min (ThuVEP group) vs 60±4 min (AE group) and hemoglobin level drop: 5±2 g/dl vs 10±7 g/dl (ThuVEP vs AE) . Catheterization time: 1 day (ThuVEP) vs 7±2 days (AE). A similar improvement in Qmax (110±10% vs 100±9%), PVR (80±3% vs 73±5%), IPSS (12±2 points) and QoL score (2±1 points) was identified in both groups (ThuVEP vs AE). The overall complication rate was 11,2% (ThuVEP) vs 31,1% (AE). No patients required hemotransfusion or reoperation. Conclusion. ThuVEP is a safe and effective surgical technique in patients with large BPH. The reduced trauma during ThuVEP, lower incidence of postoperative complications and the efficiency of ThuVEP, highlights the need for wide implementation of laser surgery. |
