Introducere. Punga de retracție (PR) este o consecință a disfuncției tubare asociată cu sechelele otitei medii exudative (OME) sau otitei medii recidivante (OMR), patologii frecvent întîlnite la copii. Simptomatologia săracă determină evoluția PR spre eroziuni osoase şi colesteatom, acestea invalidizînd copilul. Scopul lucrării. Studiul indicațiilor contemporane a timpanostomiei (TT) în tratamentul PR a pars tensa la copii şi evidențierea caracteristicilor urechii medii (UM) la pacienții supuşi acestui tip de tratament. Material şi metode. Reviul literaturii de specialitate şi studiul retrospectiv serie de cazuri. Studiul a fost efectuat conform bazelor de date a 38 pacienți cu OME sau OMR diagnosticați cu PR a pars tensa pe parcursul otomicroscopiei (clasificarea Bluestone, 2007), care au fost supuşi TT cu T tub în clinica ORL pediatrică republicană. Rezultate. Conform studiului TT a fost efectuată la 23,7% cazuri de PR gradul (gr. I), 60,5% cazuri de PR gr. II şi 15,8% cazuri de PR gr. III. Preoperator 100% copii aveau surditate de transmisie şi în 45% cazuri era prezent exudatul în UM. La 3 luni după intervenție în 87% cazuri auzul era normal, iar la 13% persista hipoacuzia uşoară. În 15,7% cazuri timpanul avea aspect normal, în 65,8% cazuri prezentă PR gr. I, iar la 18,5% cazuri acesta avea PR gr. II. Concluzii. (1) Timpanostomia este indicată în tratamentul PR a pars tensa gr. I-III asociată cu exudat în UM. (2) Auzul şi aspectul timpanului s-au îmbunătățit după efectuarea TT cu T tub.

Retraction pocket (RP) is a consequence of Eustachian tube dysfunction associated with sequels of otitis media with effusion (OME) or recurrent otitis media (ROM), pathologies frequently encountered in children. Scanty symptoms lead to evolution of RP to bone erosions and cholesteatoma, disabling the children. Objective of the study. To study contemporary indications of tympanostomy (TT) in the treatment of RP of pars tensa in children and highlight the characteristics of the middle ear (ME) in patients subjected to this type of treatment. Material and methods. Literature review and retrospective case series. The study was performed according to databases of 38 patients with OME or ROM diagnosed with RP of pars tensa during otomicroscopy (Bluestone classification, 2007), who were subjected to TT T-tube insertion in the republican otorhinolaryngology pediatric clinic. Results. According to the study, the TT was performed in 23.7% of 1st degree RP cases, 60.5% of 2nd degree RP and 15.8% of 3rd degree RP cases. Preoperatively 100% of children had transmission deafness and in 45% of cases the effusion was present. 3 months following the operation in 87% of cases the hearing was normal, and slight hypoacusis persisted in 13% of cases. In 15.7% of cases the tympanum had normal aspect, 1st degree RP was present in 65.8% of cases and 2nd degree RP was recorded in 18.5% of cases. Conclusions. (1) Tympanostomy is indicated in the treatment of 1st -3rd degree RP of pars tensa associated with effusion in ME. (2) The hearing and the aspect of tympanum improved after performing TT T-tube insertion.
