Background. Bone is a frequent site of metastases in advanced cancer. The treatment of bone metastases aims to control pain, prevent and resolve tumoral spinal cord compression and to anticipate or stabilize pathological fractures in the peripheral skeleton. Objective of the study. Evaluation of outcomes after surgical treatment of bone metastases, centered on tumor related factors and patient, timing and strategy of surgery, clinical outcome and complications. Material and Methods. This descriptive study considers recent advances in surgical treatment of the bone metastases. Variables included the different primary tumors, the metastatic load at the time of operation, the surgical margin, and the different techniques of surgical reconstruction. Results. The coverage of surgical treatment for patients with bone metastases varies between 12-15%. The 1-year survival of surgically treated patients with bone metastases exceeds 50%, and at 3 years it is about 25%. Essential prognostic factors: primary tumor histotype; the metastatic skeletal load; metastases to internal organs; development of pathological fracture. Reconstructive surgical treatment of bone metastases increases life expectancy, but the margins of surgical excision do not have a definite prognostic importance. Conclusion. Despite the advanced malignancy, clinical results indicate that treatment of pathological fractures is a safe way to improves life expectancy and the quality of life of the cancer patient. |
