Background. Nasal polyposis (NP) - a multifactorial pathology with negative impact on quality of life. The pathogenesis of NP has not yet been fully elucidated. This limits the pathogenic treatment. Recent studies suggest that nitric oxide (NO) and its metabolites are involved in pathophysiological events of NP. Objective of the study. To evaluate the serum and tissue levels of NO and NO3- (nitrate) in patients with and without NP and to establish the relationship between NO level and nasal obstruction. Material and Methods: 82 recruited patients were divided into case group (N1=41) - patients with NP and control group (N2=41) - patients with septal deviations and turbinate hypertrophies. Blood and tissue specimens were taken from all patients. Serum and tissue concentration of NO and NO3- was measured by spectrometric method. Statistics: Mann-Whitney U test, independent sample T-test, Pearson's correlation r. Results. There were no statistically significant differences between mean serum concentration of NO (t(78)=1.635, p=0.106) and NO3- (U=788, p=0.908) in study groups. There were significant differences between mean tissue concentration of NO and NO3- in study groups. Compared to control group, case group had significantly lower NO levels (U=171.5, p<0.0005) and NO3- levels (U=318, p<0.0005) in tissue specimens. Analysis of the Pearson's correlation coefficient for NO level and nasal obstruction showed the negative moderate correlation (r=-0.4, p<0.001). Conclusion. The serum level of NO and NO3- does not add any value to NP management. Tissue level of NO in NP seems to be dependent on the degree of obstruction of osteomeatal complex. NP environment is characterized by abnormalities in NO metabolism. NO and NO3- may be important in the pathogenesis of NP. |
