Одним из перспективных методов стимулирования пластичности головного мозга у пациента с нарушением двигательной функции руки после инсульта является применение интерфейса мозг-компьютер и экзоскелет кисти (ИМК + экзоскелет кисти), осуществляющего регистрацию биоэлектрической активности мозга, связанную с воображением движения (ВД) – разжимания кисти и преобразованием его по механизму обратной связи в команду к внешнему устройству – экзоскелету кисти. Оценка динамики двигательной функции руки проводилась в ранний, поздний восстановительный и резидуальный периоды инсульта с помощью международных двигательных шкал (Fugl-Meyer и ARAT) и регистрации кинематического портрета пациента – произвольные изолированные движения по каждой из семи степеней свободы паретичной руки. Для численной оценки ВД принималось среднее из максимальных вероятностей распознавания трех ментальных задач. Получены данные по 41 пациенту основной группы, 22 контрольной, 40 группы сравнения. По окончанию тренингов только в основной группе отмечалось клинически значимое улучшение по шкале Fugl-Meyer, ARAT в дистальных и в проксимальных отделах паретичной руки. В основной группе пациентов, получающих задания на ВД кисти, с высокой вероятностью распознавания динамика улучшения двигательной функции руки в позднем восстановительном периоде инсульта была достоверно выше, чем у пациентов с низкой вероятностью распознавания, (p<0,05). Применение ИМК + экзоскелет кисти после инсульта способствует улучшению двигательной функции паретичной руки. Лучшие результаты по изменению двигательной функции руки показали пациенты с высокой способностью к кинестетическому воображению. One of the promising methods of brain plasticity stimulation in post-stroke patients, who have dysfunctional arm motor abilities, is the application of brain-computer interface and hand exoskeleton (BCI + hand exoskeleton) which registers bioelectric activity of the brain related to the imagining of the movement (IM) – extension of the fingers and transforming this activity (according to the feedback mechanism) into a command for an external device – hand exoskeleton. The evaluation of the dynamics of hand motor functions was conducted in early and late rehabilitation periods as well as a residual period of stroke according to the 1) international movement scales (Fugl-Meyer and ARAT) and 2) registration of kinematic portrait of the patient, i.e. voluntary isolated movements according to seven degrees of freedom of the paretic hand. For numerical IM evaluation we used a mean of maximum probability of recognition of three mental tasks. 41 patients of the main group, 22 in the controlled group, and 40 in the comparison group were included in the analysis. By the end of the training sessions only the main group shoed clinically important improvements following the Fugl-Meyer and ARAT scales in distal and proximal joints of a paretic hand. The dynamics of the improvement of hand motor functioning in the late post-stroke rehabilitation period were significantly higher in those patients of the main group, who exhibited a higher ability for IM than the patients who had lower ability for IM (p<0.05). Biomechanical analysis in patients with higher level of IM showed an increase of strength of the muscles controlled the movements of the shoulder, elbow and wrist joints of paretic hand. The increase of muscle strength in the patients with low ability for IM was much lower. The use of BCI + hand exoskeleton in post-stroke patients helps improve motor functions of the paretic hand. The best results in improvement of hand motor functions were obtained in patients with a high ability for kinesthetic imagination. The probability of identification may serve as an index of quality of the imagined movement and characterize the intensity of training.
