Background. The Tetralogy of Fallot (TOF) clinic can range from a subtle to severe cyanosis. TOF is treated surgically through a total correction or through a temporary palliative shunt. However, with the advancement of Interventional Cardiology and imaging, endovascular procedures are explored. Objective of the study. The objective is to research, analyze, and compare existent endovascular repair alternatives and surgical palliative procedures for TOF, and to determine whatever it is a necessary procedure and a feasible one. Material and Methods. For this literature review there were searched and selected studies on the topic, published in the last 15 years. The search was conducted using the PubMed database. Results. Analyzed studies confirm that endovascular surgery is effective in palliation of TOF and is relatively superior to the Blalock-Taussig shunt in selected patients with present patent ductus arteriosus (PDA) and infundibular pulmonary stenosis. A successful balloon pulmonary valvuloplasty improves pulmonary blood flow with an increase of blood oxygenation up to 91.2 ± 6.6 and induces growth of pulmonary arteries. Patients with a PDA stent registered less complications, shorter intensive care unit stay, and larger, more symmetrical pulmonary arteries. Conversely, the study is limited due to inconsistency in the data found in the studies included in this literature review. Conclusions. All these studies confirm that non-surgical alternatives in palliation of severe TOF with cyanotic spells in neonates and children, who are not eligible for a total repair or a palliative surgical shunt, are effective and safe |
