Background. Worldwide studies report conclusive data on non-surgical treatment of spleen injuries, including in children. At the same time, the percentage of surgical resolution in spleen injuries lesions is 45-60%, not being scientifically compliant. Objective of the study. To demonstrate the clinical-paraclinical particularities in spleen lesions in children resolved conservatively, non-surgically. Material and Methods. When the treatment criteria, we evaluated 39 children, delimited in 2 groups, aged 1 month-18 years with liennial lesions. The results of the group of 18 patients, examined clinically, paraclinically (ultrasound, CT)showed stable hemodynamics, without data of hemorrhage. They These patients benefited from conservative treatment for 14 days, evaluating in dynamics the data of hemodynamics, biohumoral, acid-base balance, urine summary. Patients were discharged with improvement of the general condition, without complications. Results. For an early diagnosis and a differentiated treatment with preservation of the spleen, antenatal diagnosis is indicated as well as in childhood stages in case of abdominal trauma or polytrauma. Conclusions. Patients with splenic trauma diagnosed early, who followed a conservative treatment according to an elaborate scheme avoided surgical resolution. |
